Amide-class local anaesthetic. Blocks voltage-gated sodium channels in the inactivated state, preventing depolarisation of neuronal axons (reversible local/regional anaesthesia) and cardiac Purkinje fibres (Class IB antiarrhythmic — suppression of ventricular ectopy). Onset 45-90 s IV bolus, 5-15 min infiltration; duration 10-20 min IV bolus, 60-120 min with epinephrine 1:200,000. Hepatic metabolism via CYP3A4/CYP1A2 to active metabolites MEGX and GX.
Local and regional anaesthesia (infiltration, line block, intratesticular, epidural, intercostal, ring block, dental); CRI adjunct for intra- and post-operative analgesia (dogs); first-line treatment of haemodynamically significant ventricular tachyarrhythmias in dogs (premature ventricular complexes, ventricular tachycardia, post-GDV arrhythmias). Cats are sensitive to systemic toxicity — use markedly reduced doses or substitute alternative therapy.
