Beta-lactam antibiotics bind penicillin-binding proteins involved in cell wall synthesis, decreasing bacterial cell wall strength and rigidity, and affecting cell division, growth and septum formation. As animal cells lack a cell wall the beta-lactam antibiotics are extremely safe. The effect is bactericidal and killing occurs in a time-dependent fashion.
Piperacillin is a ureidopenicillin, classified with ticarcillin as an antipseudomonal penicillin. It is reserved for life-threatening infections (e.g. endocarditis or septicaemia) where culture and sensitivity testing predict a clinical response. These include infections caused by Pseudomonas aeruginosa and Bacteroides fragilis in neutropenic patients, although it has activity against other Gram-negative bacilli including Proteus.
