Linezolid is an antibiotic in the oxazolidinone class (synthetic drugs). It is bacteriostatic with a unique mechanism of action. It inhibits protein synthesis by binding to a site on the bacterial 23S ribosomal RNA of the 50S subunit. This prevents formation of the 70S ribosomal unit; therefore protein synthesis is inhibited. Linezolid has good penetration into cells and extracellular fluid. Urine concentrations are high enough to inhibit urinary tract pathogens. In dogs the pharmacokinetics are similar to humans. Linezolid is active against streptococci and staphylococci and is indicated for treatment of infections that have become resistant to other drugs, particularly when there is resistance to the beta-lactam antibiotics (penicillins, ampicillin derivatives, and cephalosporins). It is not indicated for gram-negative infections. Linezolid is indicated for treatment of methicillin-resistant and oxacillin-resistant strains of Staphylococcus (e.g., MRSA and MRSP) and drug-resistant Enterococcus spp. Because of the high cost of linezolid, it is not used for routine infections.
