Grapiprant is a piprant-class prostaglandin E2 (PGE2) EP4 receptor antagonist. Unlike traditional NSAIDs that inhibit cyclooxygenase (COX), grapiprant selectively blocks the EP4 receptor, which is the primary mediator of PGE2-driven inflammation and pain signaling in osteoarthritis. This targeted mechanism preserves other prostanoid pathways, potentially offering a better GI and renal safety profile compared to COX-inhibiting NSAIDs.
