Norepinephrine (noradrenaline) is an endogenous catecholamine and potent alpha-1 and alpha-2 adrenergic agonist with modest beta-1 activity. Produces powerful vasoconstriction (increased SVR and blood pressure), with mild positive inotropic effect. Minimal beta-2 activity preserves pulmonary vascular tone.
First-line vasopressor for distributive shock (septic shock) refractory to fluid resuscitation. Preferred over dopamine for septic shock in current RECOVER/SIRS guidelines.
